ESTABLISING CARE VISITS FOR CHILDREN
The establishing Primary Care Appointment is your first primary care visit with one of our providers. It is a very important appointment. We reserve a one-on-one time for you with one of our providers. The goal of the establishing care visit is for you to meet our team of Providers, ask questions and provide us with all your child’s past and current medical conditions. We also need to know all the specialists your child may have seen. We ask that you bring all the bottles of all medications your child takes if any.
We ask that you come at least 15-30 min prior to the Establishing Primary Care visit.
