Pedsplus and Centercare accept most insurance plans.

Patients are encouraged to call their insurances prior to being seen.

Please provide your current insurance information when you schedule your appointment and bring proof of your insurance to each visit.

If you are a new patient coming to establish Primary Care call your insurance to ensure we are in -network before coming to establish primary care with us. You may give one of our providers’ name, Dr Michael Perraut, Dr Laurore Jean Y. to ensure we are in-network with your insurance.

High Deductible Plans – Co-Insurances

Patients with High Deductible Insurance or who only have Co-Insurances and whose benefits have been verified and have not met their deductible, will be required to pay $25.00 towards the services rendered at each non-preventive care visit.

If, during the course of a preventive exam, services related to a new or chronic condition are completed, your insurance company may apply a copay to the visit. You will be responsible for this copay and any corresponding charges.

PREVENTIVE CARE VISIT-Why did you get a bill for a preventive care visit?

A preventive care visit with your doctor focuses on your overall health and how to stay healthy. But a preventive visit may turn into an office visit that costs you money.

When does a preventive visit become an office visit?

A preventive care visit is different from an office visit:

  • The purpose of a preventive visit is to review your overall health, identify risks and find out how to stay healthy. Your insurance plan may cover 100% of a preventive visit when you see a provider that is in your plan network.*
  • The purpose of an office visit is to discuss or get treated for a specific health concern or condition. You may have to pay for the visit as part of your deductible, copay and/or coinsurance.
  • If you schedule a preventive care visit and ask the provider about a specific health concern or condition,  the provider discussed or treated a specific health concern, condition or injury, you request medication refill for chronic medical problems, you request referral for a medical condition, the provider reviews lab work, X-rays or additional tests related to a specific health concern, condition or injury, your insurance may charge you a copay.

Preventive Care Visit Services:

Most insurances cover 100% for the services below

  • Check your weight, height, temperature, blood pressure and pulse.
  • Listen to your heart and lungs.
  • Check your ears, eyes, throat, skin and abdomen.
  • Various immunizations
  • Various cancer screenings, such as for breast, colorectal, cervical and prostate cancer
  • Certain blood tests to check such things as cholesterol or blood sugar.

Proper preventive care is important to help you live a longer, healthier life. A preventive checkup can help prevent disease before it starts and detect problems early, before they cause serious illness.
