ESTABLISH PRIMARE CARE WITH US
The establishing Primary Care Appointment is your first primary care visit with one of our providers. It is a very important appointment. We reserve a one-on-one time for you with one of our providers. The goal of the establishing care visit is for you to meet our team of Providers, ask questions and provide us with all your past and current medical conditions. We also need to know all the specialists you may have seen in the past as well as currently. We ask that you bring all the bottles of all the medications you take.
ANNUAL PHYSICAL EXAMS AND ROUTINE CHECKUPS
Life gets busy and it is a challenge to give your body the attention it needs, especially when it seems that nothing is wrong. However, regular primary care visits are the best preventive measure against developing chronic health problems like asthma, heart disease, diabetes, and preserving your long-term health. Centercare offers Primary Care services to patients and helps guides you to optimal mental and physical health.
Other conditions you may avoid or detect early with a preventive health visit include heart disease, cancer, chronic kidney disease, Alzheimer’s disease, gastrointestinal diseases and obesity, etc…

WEIGHT MANAGEMENT
Obesity is a serious, chronic disease known to reduce life span, increase disability and lead to many serious illnesses, including diabetes, heart disease, stroke and more. Excess weight is caused by interaction of genetic and environmental factors, which include metabolic and behavioral components. Because of the complexity of weight loss, gain and maintenance, promises of quick and effortless weight loss are worthless. A sedentary lifestyle is a significant barrier to weight loss and promotes further weight gain. Losing weight requires burning more calories than the body takes in by either reducing caloric intake, increasing calorie expenditures, or preferably both. Achieving and maintaining even a modest weight loss can reduce the severity of illnesses associated with obesity.
The provider at Centercare can help primary care patients lose weight but encouraging and educating them about diet, exercise as well as consequences of obesity. The providers may also help primary care patients lose weight by prescribing them medications indicated for weight loss if the patients have other comorbidities associated with obesity.
Effective weight management involves behavioral modifications which is a lifelong commitment an includes a least two components:
#1: Healthy Eating. Healthy eating emphasizing a reduction in total calories, a lowered carbohydrate process or ultra processed food consumption and an increase intake of vegetables, fruits, whole grain and an increasingly frequent and regular physical activity of at least moderate intensity.
#2: Medical and Pharmacological intervention. This is an option as previously stated for individuals with more serious cases of overweight and obesity. These interventions, combined with a plan of healthy eating and physical activities, should be utilized in conformance with applicable Treatment and Guidelines.

In our weight loss program, we do encourage you to see a registered dietitian, and we will refer you to a bariatric program if you have not been effective at losing weight.
